Service Description

This appointment includes a 25-minute visit plus 5 minutes for care coordination. *Standard follow-up visits are 30 minutes. If a 45 or 60-minute follow-up visit is needed, this will be discussed with you at your appointment. Pricing for Follow-Up Visits: 30 minutes (25-minute visit, 5-minute care coordination): $250​ 45 minutes (40-minute visit, 5-minute care coordination): $375 60 minutes (50-minute visit, 10-minute care coordination): $500 Additional time (if available) billed in 15-minute increments ($125/15 min) Form Completion: $25/form; includes prior authorizations (request in patient portal)


Cancellation Policy

Once your appointment has been confirmed, you should expect 2 emails; an email from the electronic health record with intake forms (if applicable) and a calendar invitation with the physical address location (if visit is in person on Thursday morning) or virtual link (if visit is at any other time). At Rising Together Healthcare, PLLC, we require at least 24 hours notice if you need to cancel or reschedule your appointment. If we do not receive sufficient notice, a missed appointment charge of $100 will be applied. However, we understand that unforeseen circumstances arise and we offer a one-time waiver of this charge per year. Our cancellation policy is in place to ensure that all of our clients receive the care and attention they need, while allowing us to effectively manage our schedule.
